What this assessment asks

  1. How likely are you to doze off when sitting quietly during the day (e.g. after lunch, or as a car passenger)?
  2. Do you feel sleepy or struggle to stay alert during your working hours?
  3. Have you caught yourself nodding off or micro-sleeping while driving or operating equipment?
  4. On a typical work night, how much sleep do you get?
  5. How often do you wake unrefreshed, or have trouble falling or staying asleep?
  6. How often do you rely on caffeine or energy drinks to get through the day?
  7. Do you work night shifts, early starts, rotating shifts or on-call?
  8. Do you work long shifts (10+ hours) or have short gaps (under 11 hours) between shifts?
  9. Does tiredness affect your mood, concentration or safety at or after work?
